Strive Health is hiring a Nurse Practitioner to lead clinical care for patients with CHF, CKD/ESKD and other chronic conditions. You will provide longitudinal, home- and telehealth-based care, collaborating with interdisciplinary teams to deliver high-quality outcomes.

The role emphasizes reducing hospitalizations, managing complex cases, and advancing patient engagement while maintaining board certification and licensure across applicable states.

Qualifikationen

• Master’s degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner or Primary Care NP with cardiology or nephrology focus.
• 2+ years experience with Electronic Health Records (EHR).
• Current state licensure.
• National Board Certification.
• DEA license or eligible within 90 days of hire.
• Current BLS or CPR Certification.

Aufgaben

• Manage a defined patient panel with accountability for quality, cost and outcomes.
• Obtain patient history, perform physical exam, order and interpret tests, and plan care.
• Incorporate CKD/ESKD guidance into assessment and care plans; support renal care transitions.
• Document diagnoses to support risk adjustment and care gaps.
• Support clinical rounds and interdisciplinary meetings.
• Provide care in home, telehealth, dialysis clinics or partner spaces.
