Position Overview

IU Health is seeking an innovative, energetic, and self-starting Clinical Integrated Care Manager Consultant Senior to lead and support Length of Stay (LOS) and Readmission reduction initiatives, along with other strategic projects. This role is pivotal in driving care redesign strategies, performance improvement, and statewide educational efforts to enhance patient outcomes, optimize resource utilization, and support IU Health’s mission of delivering exceptional, patient-centered care.

Key Responsibilities

• Lead and support Length of Stay and Readmission initiatives, leveraging data to identify opportunities and implement impactful solutions.
• Facilitate the local adoption and integration of care redesign strategies aligned with organizational goals.
• Execute initiatives grounded in data analysis; lead performance improvement projects across inpatient and outpatient settings.
• Assist in developing and refining strategic plans for the system, ensuring processes are sustainable and well-documented to support ongoing business needs.
• Guide and support the local implementation of care redesign methodologies, ensuring consistency and best practice adoption.
• Collaborate with clinical and operational teams to develop clinically based performance analytic tools, reports, and dashboards.
• Regularly report progress, clinical performance metrics, and improvement opportunities to key stakeholders, including leadership teams.
• Partner with IU Health system and regional leaders to develop and implement regional performance improvement strategies, particularly in LOS reduction and readmission rates.
• Lead IU Health’s statewide education team, providing monthly educational offerings, training, and support to team members across regions.

Qualifications

• Bachelor's Degree required; Master's Degree preferred.
• 5-7 years of relevant clinical experience, including 3-5 years of program management experience.
• Previous inpatient care management experience is preferred.
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
• Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC) RN license required.
