Job summary

Stonespring of Vandalia is seeking a Clinical Intake and Referral Specialist to support admissions and census goals. You will work with the Admissions Coordinator and Case Management to verify eligibility, gather documentation, and communicate referrals.

Key duties include handling PCC database entries, tours for inquiries, and data entry of referrals and admission paperwork. The role requires strong typing skills and a bachelor's degree or nursing license.

Qualifications

• Must type 55 wpm or more.
• Data entry skills required.
• Licensed nurse and/or Bachelor’s degree required.

Responsibilities

• Provide facility information and tours per sales process standards.
• Communicate referral date to Admissions Coordinator.
• Maintain PCC database of referrals and outcomes.
• Maintain sales reports tracking census building efforts.
• Maintain logs for completion of Carespring reports and functions.
• Work with residents and families to assess fit for facility.
• Coordinate with Case Management for verifications and pre-certifications of insurance.
• Complete all admission paperwork, including financial requirements.
• Assist facility leadership in other duties as assigned.
• Data entry of referrals and admission documents.
• Varying weekend on-call rotations.
