Edward M. Kennedy Community Health Center in Massachusetts seeks a Clinical Informatics Lead functioning as a Primary Care Provider and EMR Specialist.

You will deliver comprehensive primary care, coordinate patient care under physician supervision, and serve as liaison between IT and clinical teams to improve EMR workflows. The role includes EMR training for providers, workflow redesign participation, and support for new regulations.

Qualifications

• Licensed Family Nurse Practitioner or Family Medicine Physician.
• Certified and willing to clinically treat all ages (pediatric to geriatric).
• Massachusetts State medical license required.
• Minimum of 2 years clinical experience in Primary Care.
• Minimum of 2 years clinical experience using EMR systems such as EPIC, MediTech, eClinical Works, or NextGen.

Responsabilités

• Deliver direct Primary Care to assigned patients and develop therapeutic plans of care for prevention, management and maintenance of optimum health.
• Provide preventive health services and health education to promote positive health behaviors.
• Provide prenatal/postnatal care with physician partnership as needed.
• Manage acute and chronic illnesses, including assessment, diagnosis, treatment and monitoring.
• Consult with physician partner or refer to sub-specialty care for complex cases.
• Provide care to children and adults within the site as assigned by the clinical supervisor.
• Maintain and update EMR records and ensure HIPAA-compliant confidentiality.
