Job summary

Mass General Brigham Pharmacy is seeking a Clinical Pharmacist to optimize medication management and patient outcomes through evaluation and collaboration with care teams.

You will oversee specialty medication access, participate in clinical committees, and provide patient counseling, with a potential hybrid on-site/work-from-home arrangement. A PharmD and pharmacist license are required, with 2 years hospital practice experience preferred.

Qualifications

• Must hold a degree from an accredited college of pharmacy.
• Current state pharmacy license is required.
• Experience (2 years) managing patients' specialty medication needs.
• Clinical pharmacist hospital practice experience (2 years) preferred in one or more specialties.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office.

Responsibilities

• Provide medication management evaluations to ensure safe and effective utilization tailored to patient needs.
• Collaborate with patients, providers, nurses and health care professionals to optimize specialty medication access.
• Identify, resolve and prevent drug therapy problems.
• Document safety events and counsel patients on medications.
• Assist with prior authorizations, drug therapy appeals and financial assistance as directed.
• Provide on-call support on a scheduled rotation.
