Job summary

You will dispense prescriptions, adjust doses based on renal function, provide drug information to physicians and patients, and participate in anticoagulation management and discharge counseling.

Qualifications

• Registered as a pharmacist in the State of Wisconsin.
• Hospital pharmacist experience desirable.
• Ability to communicate clearly with patients and co-workers.
• Willingness to follow established policies and procedures.

Responsibilities

• Performing duties as a staff pharmacist (dispensing and clinical) at Edgerton Hospital and Health Services.
• Monitoring drug therapy for appropriateness of drug, route and duration of therapy.
• Adjusting doses for all medication as appropriate based on renal function.
• Screening for drug interactions, incompatibilities and contraindications.
• Providing drug information to physicians, nurses, allied health professionals, and patients.
• Providing clinical pharmacy services and participating in pharmacokinetic services.
• Monitors anticoagulation therapy including warfarin management and discharge medication teaching.
