Pharmacist

Our Pharmacist reports to our Director of Pharmacy and supports Edgerton Hospital by:

• Performing duties as a staff pharmacist (dispensing and clinical) at Edgerton Hospital and Health Services.
• Monitoring drug therapy for appropriateness of drug, route and duration of therapy.
• Adjusting doses for all medication as appropriate based on renal function.
• Screening for drug interactions, incompatibilities and contraindications.
• Recognizing potential adverse drug effects and intervenes as appropriate.
• Providing drug information to physicians, nurses, allied health professionals, and patients.
• Functioning within the framework of established policies and procedures of Edgerton Hospital and Health Services.

Additional Duties

• Continually strives to meet and exceed customer service standards in all interactions with customers, which include patients, visitors, other hospital employees and the medical staff. Demonstrates a commitment to professional accountability and growth.
• Interprets and processes Computerized Physician Order Entry medication orders
• Compounds sterile and non-sterile medications using proper pharmacy technique - will be trained if not done previously
• Maintains patient profiles
• Dispenses prescriptions and monitors patient for appropriate therapy and adverse effects
• Provides drug information to hospital staff.
• Provides clinical pharmacy services, assists in development and implementation of pharmacokinetic services, and collects information for medication usage evaluations.
• Monitors anticoagulation therapy including participation in Pharmacy dosed warfarin management, all anticoagulation management, and discharge medication teaching
• Consult patients on discharge medications when required.
• Organize daily operational duties.
• Supervises Pharmacy Technician(s) completing distributive tasks.
• Participate in in-service activities as needed.
• Follows hospital and department policies.

Licensure

• Registered as a pharmacist in the State of Wisconsin

Experience

• Hospital pharmacist experience desirable.

Age-Specific Competencies

• Ability to demonstrate the knowledge and skills necessary to provide pharmaceutical care to all ages, including neonate, pediatric, adolescent, adult and geriatric patients.

Essential Technical/Motor Skills

• Ability to speak clearly to communicate oral drug orders accurately and communicate with patients and co-workers. Ability to answer phone calls and communicate messages clearly, either verbally or written. Precise hand/eye coordination to enter data into the computer, use calculator and copy machines.
• Precise hand/eye coordination for the following: removal of drugs from vials or bottles, counting and measuring of drugs, relocation of drugs from one container to another, weighing of drugs and other materials using precise delicate balances, manipulation of hypodermic needles/syringes.

Interpersonal Skills

• Ability to communicate, verbally and in writing, effectively and develop positive interactions with staff, co-workers, patients and others to resolve problems effectively.
• Ability to be flexible, reliable, courteous, assertive, energetic, enthusiastic and have a positive attitude in order to maintain a harmonious and efficient work environment.

Essential Physical Requirements

• The ability to move from one area of the pharmacy and/or hospital to another.
• The ability to lift, push and carry various objects.
• Requires bending, stooping, climbing on foot stool, and may require lifting boxes weighing up to 20 pounds; stand or walk for several hours at a time; may need to climb stairs from one floor to another.
• Maintains good personal hygiene.

• Must be able to follow routine daily schedule without constant reminders.
• Ability to adapt to a varied, inpatient/outpatient routine.
• Ability to organize work, train, and oversee work of technicians. Must be able to manipulate numbers, calculate doses, and interpret lab data.
• The ability to consistently perform accurately. Ability to assess appropriateness of drug therapy and explain it to patients and other health professionals.
• Requires higher level of mental faculties, accompanied by short and long term memory.
• Must remain currently knowledgeable about drugs, federal and state regulations, pharmacology, etc.

• The ability to evaluate situations and to respond quickly to those situations.
• Must be able to resolve problems involving patients, families, physicians and staff.

Essential Sensory Requirements

• Requires sight to identify drugs, often requiring ability to distinguish color.
• Ability to visually scan/proof typed work and computer information.
• Ability to communicate via telephone to a variety of individuals - patients, physicians, other clinic personnel, insurance companies, etc.
• The ability to see, read, and file patient information.

Exposure To Hazards

• May have exposure to chemotherapy agents and other hazardous medications if preparing these products.
• May be exposed to needle puncture and glass cut wounds when preparing sterile products.
• May be exposed to blood when processing used drug trays.
• May be exposed to irritating chemicals during medication preparation or cleaning procedures.
