Job summary

Via Elegante Assisted Living in Tucson, AZ seeks a Clinical Director | Care Concierge to lead clinical operations and advocate for residents and families. You will coordinate care with physicians, pharmacies, labs, and home health providers, and oversee medication management and clinical documentation.

You'll build a high-performing team, coach caregivers, and participate in on-call rotations to ensure compassionate, compliant care across memory care and assisted living services.

Qualifications

• Must hold active, unrestricted Arizona RN, LPN, or LVN license.
• Minimum 3+ years nursing experience in assisted living, memory care, skilled nursing, home health, or similar.
• Minimum 1+ year clinical leadership or supervisory experience.
• Minimum 1+ year hands-on dementia care and medication administration experience.
• Experience with EHR documentation and eMAR systems.
• Current CPR and First Aid certification.
• Valid Arizona fingerprint clearance card or ability to obtain one in required timelines.
• Valid driver's license and access to a private vehicle.

Responsibilities

MAKE AN IMPACT

As our Clinical Director | Care Concierge, you’ll lead clinical operations while serving as a trusted advocate and care coordinator for our residents and families. Through our partnership with Centered Care, you’ll use proactive care practices, technology, and clinical insight to identify needs early and help ensure residents receive the right support at the right time.

You’ll have the opportunity to:

• Build strong relationships with residents, families, physicians, and healthcare partners.

• Coordinate care with physicians, specialists, pharmacies, labs, home health, hospice, and therapy providers.

WHAT YOU’LL BRING

• Active, unrestricted Arizona RN, LPN, or LVN license.

• 3+ years of nursing experience in assisted living, memory care, skilled nursing, home health, or comparable senior care.

• 1+ year of clinical leadership or supervisory experience.

• 1+ year of hands-on dementia care and medication administration experience.

• Experience with EHR documentation and eMAR systems.

• Current CPR and First Aid certification.

• Valid Arizona fingerprint clearance card or ability to obtain one within required timelines.

• Valid driver's license and access to a private vehicle for business use.

Preferred: Experience coordinating care across healthcare providers, behavioral health or dementia care experience, care management technology, Arizona assisted living regulations, or Arizona Assisted Living Facility Manager certification.
