Via Elegante Assisted Living & Memory Care seeks a Clinical Director | Care Concierge to lead clinical operations and act as a trusted advocate for residents and families. You will coordinate care with physicians, pharmacies, labs, home health, hospice, and therapy providers, while driving quality initiatives and staff development.

Qualifications

• Active, unrestricted Arizona RN, LPN, or LVN license.
• 3+ years of nursing experience in assisted living, memory care, skilled nursing, home health, or comparable senior care.
• 1+ year of clinical leadership or supervisory experience.
• 1+ year of hands-on dementia care and medication administration experience.
• Experience with EHR documentation and eMAR systems.
• Current CPR and First Aid certification.
• Valid Arizona fingerprint clearance card or ability to obtain one within required timelines.
• Valid driver's license and access to a private vehicle for business use.

Responsibilities

• Lead and develop a high-performing clinical team.
• Oversee individualized resident care and service plans.
• Build strong relationships with residents, families, physicians, and healthcare partners.
• Coordinate care with physicians, specialists, pharmacies, labs, home health, hospice, and therapy providers.
• Lead interdisciplinary team meetings and care conferences.
• Oversee medication management, clinical documentation, and quality initiatives.
• Coach and educate caregivers and clinical team members.
• Support dementia care, resident transitions, and complex healthcare needs.
• Serve as a clinical leader during urgent situations and participate in the director on-call rotation.
