Job summary

Aventura at West Park is seeking an experienced Wellness Director to lead clinical and wellness services in our Assisted Living community. You will oversee nurses, caregivers, and care aides, build a strong clinical team, and drive a comprehensive wellness program that enhances resident well-being and family confidence.

Qualifications

• Active RN or LPN license.
• Assisted Living, senior living, or related healthcare experience preferred.
• Clinical leadership or supervisory experience.
• Strong knowledge of resident assessments, care planning, medication management, and regulatory compliance.

Responsibilities

• Lead, support, and supervise nurses, medication technicians, caregivers, and care aides while promoting accountability and teamwork.
• Complete initial and ongoing resident health assessments and identify changes in condition or level-of-care needs.
• Develop, review, and update resident service and care plans to ensure needs are addressed.
• Coordinate resident services with physicians, pharmacies, home health, hospice, therapy, and other providers.
• Serve as a trusted clinical resource for residents and families, providing updates and communicating changes in condition or care needs.
• Maintain compliance with state regulations, healthcare standards, infection-control practices, and community policies.
• Participate in new resident assessments, admissions, hospital returns, and transitions of care.
• Partner with the Executive Director to support resident satisfaction, staffing, retention, and overall community success.

What You'll Do

• Clinical Leadership: Lead, support, and supervise nurses, medication technicians, caregivers, and care aides while promoting accountability and teamwork.

• Individualized Care Plans: Develop, review, and update resident service and care plans to ensure each resident’s needs are appropriately addressed.

• Family Communication: Serve as a trusted clinical resource for residents and families, providing updates and communicating changes in condition or care needs.

• Admissions & Transitions: Participate in new resident assessments, admissions, hospital returns, and transitions of care to help ensure appropriate placement and continuity of services.


• Collaborative Leadership: Partner closely with the Executive Director and community leadership team to support resident satisfaction, staffing, retention, compliance, and overall community success.

What We're Looking For

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license


• Assisted Living, senior living, or related healthcare experience preferred


• Clinical leadership or supervisory experience


• Strong knowledge of resident assessments, care planning, medication management, and regulatory compliance


• Excellent communication, organization, and clinical judgment


• A leader who is visible, approachable, compassionate, and accountable
