Qualifications

• Bachelor of Dietetics with ACEND accreditation.
• RD exam passed and registered to practice.
• Experience applying CMS guidelines in care plans.

Responsabilités

• Complete nutrition assessments at enrollment and semi-annually.
• Document PES statements and develop nutrition diagnoses.
• Develop and implement individualized nutrition interventions.
• Coordinate care with the IDT and monitor progress.
