Responsibilities:
- The Clinical Dietitian II assesses the nutritional status of the patient using recognized tools of assessment, and recommends and counsels patients, family, and/or caregivers on the appropriate nutrition plan to improve the patient’s disease or condition management. The Clinical Dietitian II supports, models, and trains Clinical Dietitian I's in the core behaviors and responsibilities of the department and organization. Performs business functions, such as management of resources in a cost-effective manner, and participates in compliance and quality performance improvement efforts to ensure that nutrition care is safe and effective. This position also assists the Clinical Nutrition Director (if applicable) with coordination of clinical nutrition services.
- Conducts patient screening in accordance with specialized clinical guidelines for those at nutritional risk. Performs nutritional assessments using age and gender appropriate standards, cultural, religious, and ethnic backgrounds; utilizing validated assessment tools resulting in nutrition-related diagnosis when indicated. Collaborates within an interdisciplinary environment to facilitate care and identify patients at nutrition risk or needing higher levels of care.
- Implements targeted nutrition interventions based on medical conditions. Provides customized nutrition education to patients and families. Participates as a member of the interdisciplinary care team. Monitors and updates patient nutrition care plans, and follows up as needed.
- Develops intervention plans including feeding routes, modes, and nutrition prescriptions tailored to patient needs, in accordance with physicians’ orders. Monitors patient responses and documents progress in medical records. Coordinates nutrition services with California Children's Services (CCS) for neonates and pediatric patients if applicable.
- Collaborates with Food and Nutrition Services to enhance patient experience, participate in menu reviews, audits, and training. Supports staff development and training sessions, including Medical Nutrition Therapy workshops.
- Provides expert nutrition care for high-acuity patients in neonatal, pediatric, oncology, renal, diabetes, and gerontology units, with order writing privileges.
- Performs other duties as assigned.
Qualifications:
- Bachelor's Degree from an accredited dietetics program (ACEND).
- At least 3 years of experience in acute care or similar settings with complex populations.
- State Licensure within 3 months of employment.
