Job summary

You will communicate nutritional data across systems, participate in health team meetings, and collaborate with Food and Nutrition Services to ensure diet orders align with the Diet Manual Handbook and care plans.

Qualifications

• Nutrition assessment using standard age, gender, culture, and validated tools.
• Interdisciplinary collaboration to facilitate care and identify nutrition risk.
• Provide customized nutrition education to patients and caregivers; monitor and reassess nutrition status.

Responsibilities

• Conduct nutrition assessments and screenings to identify patients at nutrition risk.
• Develop and implement nutrition interventions and education plans.
• Collaborate with Food and Nutrition Services to ensure diet orders meet manuals and care plans.
• Monitor patient response to interventions and update care plans accordingly.
• Participate in peer reviews and chart audits.
