Qualifications

• Registered by the Commission on Dietetic Registration.
• Certification/licensure in good standing within the state of practice.
• Knowledge of state survey process and nutrition care regulations (preferred).
• Strong organizational and communication skills.
• Maintain a positive attitude and effective communication with occupants, administration, and HCSG leadership.
• COVID-19 vaccination policy compliance as required by state and customers.
• If multi-site RD, may require a valid Driver's License.
• RD-Eligible and provisional state licensure accepted in certain locations.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings, labs, weight trends, intake and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
• All other duties as assigned.
