Job summary

Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutrition services at a long-term care facility. You will assess residents' nutrition needs, develop and document care plans, and provide nutrition education to residents, families, and staff.

Qualifications

• Registered by the Commission on Dietetic Registration.
• State certification/licensure in good standing.
• Knowledge of nutrition regulations and CMS/state standards.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical findings and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and well-being.
• Ensure CMS/state/federal regulatory adherence and company standards.
• Provide coverage across multiple buildings as needed.
• All other duties as assigned.
