Job summary

FutureCare seeks a Registered Dietitian to join our interdisciplinary team, delivering personalized nutrition care through timely assessments and individualized interventions. The role spans facilities including VitalStrong Rehabilitation, long-term care, and dialysis services.

The candidate should hold a Maryland RD license, a Bachelor’s in Nutrition/Dietetics, and have at least 2 years of experience in a skilled nursing setting with a commitment to evidence-based care.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferred.
• Previous experience working in a skilled nursing facility is highly desired.
• Knowledge of the Nutrition Care Process and ability to apply all steps of the process.

Responsibilities

• Work with Interdisciplinary Team to ensure residents receive optimal clinical care.
• Provide nutrition services and medical nutrition therapy aligned with patients' medical needs.
• Assess nutritional status using validated screening tools.
• Complete timely nutrition and MDS assessments as required.
• Plan, develop, implement and monitor care plans for each resident.
• Provide nutrition recommendations for patients relying on enteral nutrition support.
• Educate patients and families on nutrition.
• Follow current standards and evidence-based practices.
• Support residents’ rights and prioritize patient-based care.

• Provide nutrition education to patients and families.
• Utilize current standards and evidence-based practices.
• Prioritize patient-based care and residents’ rights.
• Complete monthly quality assurance and process improvement audits.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferred.
• Previous experience working in a skilled nursing facility is highly desired.
• Knowledge of the Nutrition Care Process and ability to apply all steps of the process.
• A strong desire to deliver care in an empathetic, compassionate way.
• Superb organizational and time management skills.
• Decision making, problem solving, and collaboration skills are a plus.
• Excellent interpersonal, written, and verbal communication skills.
