Job summary

FutureCare is seeking a Clinical Dietitian to provide personalized nutrition care to residents within a top workplace environment in Maryland. You will perform nutrition assessments, develop care plans, and deliver medical nutrition therapy as part of an Interdisciplinary Team.

You will use validated screening tools and MDS assessments, plan and monitor nutrition care, and educate residents and families on nutrition. A Maryland RD license and strong collaboration skills are essential.

Qualifications

• Currently licensed Registered Dietitian in Maryland.
• Experience with Nutrition Care Process and applying all steps.
• Strong organizational, time management and communication skills.

Responsibilities

• Work with Interdisciplinary Team to deliver optimal clinical care.
• Provide medical nutrition therapy aligned with patient medical needs.
• Assess nutritional status using validated screening tools and MDS assessments.
• Plan, develop, implement, and monitor nutrition care plans for residents.
• Provide nutrition recommendations for enteral nutrition where required.
• Educate patients and families on nutrition and care plans.
• Apply current standards and evidence-based practices.
• Prioritize patient rights and individualized care.
• Complete monthly quality assurance and process improvement audits.

Responsibilities

• Work closely with the Interdisciplinary Team to ensure residents receive the best clinical care possible.
• Provide nutrition services and medical nutrition therapy integrated with the patient's medical needs.
• Assess and identify the nutritional status of residents using validated screening tools.
• Complete timely nutrition assessments and Minimum Data Set (MDS) assessments as required.
• Plan, develop, implement and monitor the nutrition plan of care for each resident.
• Provide nutrition recommendations for patients reliant on enteral nutrition support.
• Provide nutrition education to patients and families.
• Utilize current standards and evidence-based practices.
• Prioritize patient-based care and residents’ rights.
• Complete monthly quality assurance and process improvement audits.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferred.
• Previous experience working in a skilled nursing facility is highly desired.
• Knowledge of the Nutrition Care Process and ability to apply all steps of the process.
• A strong desire to deliver care in an empathetic, compassionate way.
• Superb organizational and time management skills.
• Decision making, problem solving, and collaboration skills are a plus.
• Excellent interpersonal, written, and verbal communication skills.
• Strong leadership skills and ability to work collaboratively with team members.
