Job summary

FutureCare Associates, Inc. is seeking a qualified Clinical Dietitian to join our interdisciplinary team in providing personalized nutrition care to residents across our facilities.

You will conduct timely nutrition assessments, develop and monitor care plans, and collaborate with physicians and care teams to optimize patient outcomes. The ideal candidate holds a Maryland RD license, a bachelor\'s in Nutrition/Dietetics, and 2+ years in a skilled nursing setting.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferred.
• Previous experience working in a skilled nursing facility is highly desired.
• Knowledge of the Nutrition Care Process and ability to apply all steps of the process.

Responsibilities

• Work closely with the Interdisciplinary Team to ensure residents receive the best clinical care possible.
• Provide nutrition services and medical nutrition therapy integrated with the patient's medical needs.
• Assess and identify the nutritional status of residents using validated screening tools.
• Complete timely nutrition assessments and Minimum Data Set (MDS) assessments as required.
• Plan, develop, implement and monitor the nutrition plan of care for each resident.
• Provide nutrition recommendations for patients reliant on enteral nutrition support.
• Provide nutrition education to patients and families.
• Utilize current standards and evidence-based practices.
• Prioritize patient-based care and residents’ rights.
• Complete monthly quality assurance and process improvement audits.
