Qualifications

Job description

Responsibilities

As our Clinical Dietitian, you will assess the nutritional status of patients using recognized assessment tools. You will recommend and counsel patients, families, and caregivers on appropriate nutrition plans to improve health and well-being.

You will work with the general patient population and those in more complex care levels under supervision to perform nutritional assessments based on age, gender, cultural, religious, and ethnic backgrounds. You will conduct routine screenings to identify patients at nutritional risk, collaborate within an interdisciplinary team, and contribute to facilitating care for higher-level needs, all while providing excellent customer service to patients, families, and caregivers.

Responsibilities:
  • Develop intervention plans including feeding routes, modes, and nutrition prescriptions tailored to patient needs, following physicians' orders.
  • Monitor and evaluate patient responses to interventions, meeting specific goals documented in the nutrition care plan promptly.
  • Update care plans as necessary and follow up with patients as needed.
  • Collaborate within an interdisciplinary team to facilitate care and identify patients at nutrition risk or requiring higher care levels.
  • Work with the Food and Nutrition Services team to enhance patient experience.
Qualifications
Minimum:
  • Bachelor's Degree with completion of an accredited dietitian program (ACEND)
  • Valid registration as a Registered Dietitian with the CDR within 3 months
