Responsibilities

• Plan, develop, implement, and monitor the nutrition care plans for residents
• Conduct timely nutrition assessments, care planning, and complete Minimum Data Set (MDS) assessments
• Communicate the nutrition plan of care effectively to the interdisciplinary team, residents, and families
• Develop and monitor nutrition programs as part of performance improvement
• Identify patients at nutrition risk via screening
• Provide nutrition education and counseling to patients and families
• Ensure proper nutritional care through assessments and interventions
• Develop nutrition education materials and care plans
• Document nutrition interventions in patient records
• Assess and evaluate patients' nutritional status and needs
• Participate in managing patient feeding as part of the Food Service Management team
• Recommend and order nutritional supplements and enteral/parenteral nutrition as needed

Qualifications

• Current Registered Dietitian license in Maryland
• Bachelor’s degree in Food and Nutrition, Dietetics, or related field
• At least 2 years of experience as a dietitian preferred
• Knowledge of the Nutrition Care Process
• Empathetic and compassionate care delivery
• Experience in a skilled nursing facility preferred
• Strong organizational, communication, and leadership skills
• Ability to counsel and educate effectively
