Qualifications

  • Active, unrestricted CDCES certification required.

Job description

Clinical Customer Success Manager (Certified Diabetes Education & Care Specialist)

SweetSpot is seeking a Clinical Customer Success Manager with a CDCES certification to play an integral role in the ongoing clinical support for our clinic partners utilizing our remote diabetes monitoring platform and service, SweetSpot. This position involves analyzing patient glucose data, providing clinical recommendations, and supporting healthcare practices in their adoption and continued use of SweetSpot. The ideal candidate will have extensive experience in diabetes care management, strong communication skills, and a passion for leveraging digital solutions to improve patient outcomes.

Responsibilities:
  1. Clinical Responsibilities:
  2. Analyze and interpret CGM and/or insulin pump reports for T1D/T2D patients through remote reviews.
  3. Provide clinical recommendations and document via EMR systems.
  4. Conduct follow-up calls with patients to discuss data reviews and regimen changes.
  5. Provide diabetes self-management education during remote reviews.
  6. Customer Success:
  7. Support the growth of the SweetSpot program, ensuring quality support and communication of success metrics.
  8. Engage with healthcare practice customers to understand needs and communicate progress.
  9. Collaborate with clinicians to resolve workflow challenges and develop solutions.
  10. Utilize CRM system (PipeDrive) for communication and task management.
  11. Qualifications:
  12. Certified RN, RD, NP, or PA with diabetes experience.
  13. Active, unrestricted CDCES certification.
  14. 5+ years of practice experience as a CDCES in diabetes care management.
  15. 5+ years of experience with CGM and insulin pump management.
  16. Bonus Qualifications:
  17. Experience with diabetes devices (Dexcom, Libre, Eversense, Medtronic, Tandem, Omnipod).
  18. Experience with EHR/EMR systems.
  19. 3+ years in the diabetes industry.
  20. Ability to manage multiple projects in a fast-paced environment.
  21. Additional Requirements:

  23. Proven success in program implementation and remote customer support.
  24. Excellent communication and interpersonal skills.
  25. Critical thinking and problem-solving skills.
