CN Guidance & Counseling Services, Inc. in New York, NY, seeks a licensed clinician to manage a caseload of 25-35 clients, conduct assessments, and develop treatment plans.

The role includes leading groups, coordinating care, and participating in discharge planning within a multidisciplinary team. The position requires a Bachelor's degree with 2 years of experience or a Master’s degree in Social Work, Mental Health Counseling, or Creative Arts Therapy, with PROS experience preferred.

JOB DEFINITION:

Utilizes various activities under supervision to serve the treatment needs of clients and help them make progress toward their goals.

EDUCATIONAL REQUIREMENTS:

Bachelor's Degree and 2 years of experience, or Master's in Social Work, Mental Health Counseling, or Creative Arts Therapy.

EXPERIENCE REQUIRED:

2 years of experience working with adults diagnosed with mental illness and co-occurring substance use, in an individual and group setting. Experience in a PROS setting preferred.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Managing a caseload of 25-35 clients, which includes (but is not limited to) the following:
   1. Conduct required assessments for new clients and on an ongoing basis with existing clients
   2. Develop treatment plans in collaboration with recipients; review and update as required
   3. Meet individually with clients to address progress/barriers toward treatment goals
   4. Maintain contact with collaterals to coordinate care/treatment of clients
   5. Coordinate discharge plans once the client is ready to move on from program
2. Facilitate therapeutic and skill-building groups
   1. Utilize evidence-based materials when facilitating groups
3. Conduct outreach as needed to promote client engagement in program services.
4. Follow documentation and billing guidelines for all individual and group services; review all documentation/billing to ensure for accuracy.
5. Work as part of a multi-disciplinary team in developing and executing interventions for optimal client care
6. Utilize clinical skills/interventions in group and individual sessions to help clients work through barriers and work toward goal acquisition; utilize supervision and ongoing continuing education opportunities to strengthen these skills/interventions.
7. Represents program at community meetings on an as-needed basis
8. Ensures a safe, secure and nurturing environment is provided and maintained for clients
9. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision‑making and to keep clients informed of progress
10. Complete QI Training during onboarding and participate in QI refresher training as needed.
