Inspira Health is seeking a Clinical Coordinator RN to partner with IMG practices, helping patients navigate our health system and coordinating their needs with organizational resources. The role supports clinical triage and transfers using protocols and clinical judgment to determine appropriate actions.

Ideal candidates hold a NJ RN license with BLS, and have 3–5 years of direct patient care experience in inpatient, rehab, sub-acute, or ambulatory care settings.

Qualifications

• Licensed RN with required NJ license and BLS certification.
• Experience in acute inpatient, rehab or ambulatory care settings preferred.
• 3–5 years direct patient care experience required.

MAJOR FUNCTION:The Clinical Coordinator RN will operate in direct partnership with IMG practices. To assist our patients in navigating the various complexities of our health system and act as a resource to coordinate the patients' needs with our organization resources. This role will act as a clinical support for the IMG practices by support their pool's and provide clinical triage. This role will complete triage transfers by utilize protocols and clinical judgement to assess and prioritize a patients need and determine the appropriate course of action.

QUALIFICATIONS

Education Experience:Associate degree required.Bachelors degree preferred. Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare.Experience in a care management role preferred.Experience in Ambulatory care preferred.3-5 years of direct patient care experience required.

Certification / Licensure

Active New Jersey Registered Nurse License is required.Active Basic Life Support (BLS) certification is required.
