Job summary

Mentor Community Services, part of Sevita, seeks a Program Services Clinical Coordinator to coordinate services, case management, behavior plans, and treatments for individuals served.

You will oversee all services, monitor quality and compliance, and build strong relationships with families and external case managers to ensure high-quality supports.

Qualifications

• Bachelor’s degree required in human services or related field.
• 1 year of relevant experience preferred.
• Current driver’s license, car registration, and auto insurance.
• Licensure and training as required by state (CPR, First Aid, Behavioral Intervention Techniques).
• QMRP/QIDDP as required by state.
• Reliable, compassionate and quality-focused attitude.

Responsibilities

• Coordinate development, review, and implementation of services, case management, behavior intervention plans, and treatments for individuals served.
• Oversee all services provided to individuals and families, monitor quality of care and contractual compliance, and regulatory program compliance.
• Support development of quality services and engaging activities meeting individual needs.
• Build and maintain relationships with families and external case managers.
