Job summary

Mentor Community Services, part of Sevita, seeks a Program Services Clinical Coordinator to coordinate services, case management, behavior plans, and treatments for individuals served.

You will oversee all services, monitor quality and compliance, and build strong relationships with families and external case managers to ensure high-quality supports.

Qualifications

• Bachelor’s degree required in human services or related field.
• 1 year of relevant experience preferred.
• Current driver’s license, car registration, and auto insurance.
• Licensure and training as required by state (CPR, First Aid, Behavioral Intervention Techniques).
• QMRP/QIDDP as required by state.
• Reliable, compassionate and quality-focused attitude.

Program Services Clinical Coordinator

Qualifications:

• Bachelor’s degree in human services or related field.
• One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc. preferred.
• An equivalent combination of education and experience.
• Current driver's license, car registration, and auto insurance.
• Licensure and training as required by state such as CPR, First Aid, Behavioral Intervention Techniques, etc.
• QMRP/QIDDP as required by state.
• A reliable, responsible attitude and a compassionate approach.
