Job summary

New Avenues to Independence, Inc. is seeking an LPN to provide clinical coordination of health services for an assigned caseload. You will report to supervision any issues affecting health, safety, or wellbeing of residents, staff, or property.

The role focuses on coordinating health appointments, observing resident progress, and ensuring accuracy in health documentation while maintaining high standards of care within our residential program.

Qualifications

  • Active Ohio LPN license
  • Minimum 1 year of LPN experience in acute and/or long-term care
  • Residential programming experience preferred
  • Must have an OH driver’s license
  • High school diploma or equivalent

Responsibilities

  • Provide clinical training, supervision and support to LPNs and Nursing Assistants and program staff
  • Coordinate and follow-up on major resident health appointments
  • Observe and chart residents' progress, behavior and incidents regularly
  • Report healthsafety concerns to supervisor and follow incident protocols
  • Educate Residential Manager/Coordinator and direct care staff with health information
  • Prepare summaries and reports in a timely, regular manner
  • Follow through on lab results, dietary, therapy, pharmacy and IHP recommendations
  • Attend annual IHP meetings and pertinent quarterly IHPs
  • Monitor staff and ensure MAR documentation accuracy and safety of delegated nursing tasks

Position overview:

This position provides clinical coordination of health services for an assigned caseload to ensure the best possible standard of care. Responsible to promptly report to supervisor any circumstances that negatively affect the health, safety, or wellbeing of residents, staff, the agency or its property and equipment.

Pay rate:

$27 per hour

Key responsibilities:
  • Provides clinical training, supervision, and support as needed to fellow Licensed Practical Nurses, (LPN’s) as well as Nursing Assistants and programmatic staff.
  • Coordinate and follow-up all major resident health appointments.
  • Observe and charts residents' progress, behavior and unusual incidents on a timely, regular, consistent basis.
  • Report unusual developments to immediate supervisor, Residential Manager/Q, Residential Coordinator and follow Major Unusual Incident reporting protocol.
  • Communicate and educate Residential Manager/Q, Residential Coordinator and direct care staff as information is gathered or as health needs warrant.
  • Assess and write summaries and reports as required on a timely, regular and consistent basis.
  • Follow through on lab results, dietary, therapy, pharmacy and IHP recommendations.
  • Attend annual IHP, Special meetings, and pertinent Quarterly IHP's.
  • Monitor trained, unlicensed direct care staff; ensure MAR timely documentation/accuracy, and check medications to ensure safety of delegated nursing program.
Requirements

Licensed Practical Nurse with current state of Ohio licensure.

  • One year of experience as an LPN in acute and/or long-term care setting; prefer minimum one year experience in residential programming of developmentally disabled; OR any equivalent combination of experience and/or education from which comparable knowledge, skills and abilities have been achieved.
  • 18yrs of age or older
  • Must have an OH driver's license.
  • High school diploma
