The ideal candidate holds an LCSW or LMSW, has experience working with individuals with disabilities, and possesses strong communication and clinical skills. The salary is competitive, ranging from $60,000 to $70,000 based on experience.

Qualifications

Responsibilities

• Participate in Life Plan meetings and develop individualized service delivery.
• Collect and maintain required documentation in compliance with standards.
• Provide individual supports and facilitate group sessions.
