Job summary

Yachad, a division of The Orthodox Union, is seeking a Clinical Coordinator to support growth for individuals with developmental disabilities. The role is full-time, based in the Five Towns and Brooklyn, with two days in the Five Towns and three in Brooklyn.

You will participate in Life Plan meetings, develop action plans, and ensure regulatory documentation is maintained. LCSW/LMSW and SIFI certification willingness are preferred.

Qualifications

• LCSW or LMSW licensure required.
• Experience working with individuals with disabilities.
• Must be an effective communicator.
• Ability to work well with others.
• Strong clinical knowledge.
• In process of taking or willingness to become SIFI certified.

Responsibilities

• Participate in Life Plan meetings with individuals, families and teams.
• Develop, implement, and update Staff Action Plans for consistent service delivery.
• Collect and maintain required annual documentation per regulatory standards.
• Provide individual supports and facilitate group sessions on social, emotional, and life skills.
• Assist with daily program operations and general program needs.
• Complete timely and accurate documentation of services and progress notes.
• Review and update Person-Specific Profiles and related paperwork.
• Participate in intake processes and follow-up as needed.
• Provide Clinical Supervision of Social Work Interns to ensure quality practice.

Position Summary:

Jewish Union Foundation is looking for a Clinical Coordinator to work closely with staff to help facilitate growth for individuals with developmental disabilities. This is a full-time position based in both the Five Towns and Brooklyn, with two days per week in the Five Towns and three days per week in Brooklyn.

Responsibilities:

• Participate in Life Plan meetings, contributing to person-centered planning with individuals, families, and interdisciplinary teams.

Salary & Benefits:

The salary range for this (full-time) position is $60,000 - $70,000, commensurate with experience.
