Job summary

Yachad, a division of The Orthodox Union, is seeking a Clinical Coordinator in New York to collaborate with staff and families in supporting individuals with developmental disabilities. This full-time role alternates between the Five Towns and Brooklyn, with two days in Five Towns and three days in Brooklyn.

You will participate in Life Plan meetings, develop action plans, collect documentation, provide individual supports, supervise interns, and ensure regulatory compliance.

Qualifications

• LCSW or LMSW license required.
• Experience working with individuals with disabilities.
• Strong clinical knowledge.
• Excellent communication and teamwork abilities.
• Willingness to pursue SIFI certification.

Responsibilities

• Participate in Life Plan meetings with individuals, families, and interdisciplinary teams.
• Develop, implement, and update Staff Action Plans for individualized service delivery.
• Collect, track, and maintain required annual documentation in compliance with standards.
• Provide individual supports and facilitate group sessions on social, emotional, and life skills.
• Assist with daily program operations and complete documentation of services and progress notes.
• Review Person-Specific Profiles and update as needs change.
• Participate in intake processes and follow-up.
• Provide Clinical Supervision of Social Work Interns to ensure quality practice.
