Job summary

Aspire is seeking a Registered Nurse to join our Scottsdale team as Clinical Coordinator, coordinating care for home health patients and supporting field clinicians. In this role you will ensure seamless communication among patients, families, and physicians, with in-office and field visits.

Qualifications

• Active Arizona RN license is required.
• Minimum 2 years of experience in home health or acute care (med-surg, tele, ER, etc.).
• Current CPR certification and TB test within 12 months.
• Valid driver’s license.
• Ability to work Monday–Friday, 8 AM–5 PM in Scottsdale office.

Responsibilities

• Conduct home visits for SOC, ROC, and Recertifications as needed.
• Triage calls from field staff and provide clinical support.
• Serve as liaison between patients, families, and physicians.
• Review and approve OASIS documentation and care plans.
• Assist with onboarding, training, and evaluations.
