Job summary

Kids for the Future in Scottsdale, AZ is seeking a Registered Nurse to provide home health nursing care to residents in their homes, under the supervision of the Administrator. The RN delivers direct and indirect nursing services within the scope defined by the Arizona State Board of Nursing.

Responsibilities include conducting resident assessments, preparing care plans, administering medications as prescribed, and coordinating care with physicians and campus staff.

Qualifications

• Active RN license in Arizona.
• One year of home health and/or retirement/assisted living experience preferred.
• Current CPR and First Aid certification.
• Ability to pass fingerprint background clearance.

Responsibilities

• Provides and documents nursing care of residents.
• Conducts resident assessments and care plans including faxing physicians and supervisory visits.
• Nursing care is guided by physician orders and the home health care plan.
• Participates in maintaining the environment of care, including equipment and other material resources.
• Provides for the emotional and physical comfort of residents.
• Observes, records, reports, and acts upon changes in resident condition.
• Follows up on resident care and progress via phone and assists in coordinating their return to campus.
• Orders and prepares medication/medisets as prescribed by a physician.
• Provides and documents triage and emergency home visits.
• Follows up on laboratory results and subsequent physician orders.
• Documents nursing care in the clinical record on a timely and accurate basis.
• Assists with residents' activities of daily living, including personal care, wheelchair escorts, and medication reminders.
• Updates the medication administration record and other clinical records as indicated.
• Assists with health promotion activities, such as influenza and blood pressure screening clinics.

Education

Registered Nurse (RN) license in Arizona

CPR certification

Position Summary:

The Registered Nurse provides home health nursing care to residents in their homes, functioning under the supervision of the Administrator. This position assists in the oversight of the Home Health department in accordance with facility and state regulatory requirements. The RN delivers direct and indirect nursing services within the scope of RN practice as defined by the Arizona State Board of Nursing.

• Nursing care is guided by physician orders and the home health care plan.

Education/Experience:

• Active Registered Nurse (RN) license in the State of Arizona, current and in good standing
• One year of home health and/or retirement/assisted living experience preferred
• Current CPR and First Aid certification
• Ability to pass fingerprint background clearance

Physical Demands:

• Able to sit, stand, bend, lift, and move intermittently throughout the workday
• Able to lift up to 40 pounds
• Able to stand and walk for extended periods
• Basic computer proficiency required
