Job Title: Clinical Coordinator – Transitional Housing

Location: Multiple Sites Across Brooklyn, Manhattan, and Queens, NY

Reports To: Assistant Program Director, Transitional Housing

Employment Type: Full-Time | Monday–Friday (Business Hours) | On-Call Availability as Needed

Job Summary:

We are seeking a compassionate and experienced Clinical Coordinator to join our Transitional Housing program. This role is pivotal in supervising a team of Case Managers and ensuring the use of best-practice clinical approaches—including Harm Reduction, Housing First, and trauma-informed care—to support clients transitioning out of homelessness. This position promotes high-quality service delivery and ensures compliance with documentation and regulatory standards.

Key Responsibilities:

• Supervise and support a team of Case Managers, providing regular, structured supervision
• Implement and promote effective case management strategies using evidence-based models such as Housing First, Harm Reduction, Motivational Interviewing, and trauma-informed care
• Oversee the development and timely documentation of individualized service plans
• Interview and assess prospective clients for transitional housing placement
• Provide clinical leadership to address client engagement, behavioral challenges, and program troubleshooting
• Foster a program culture that emphasizes permanent housing placement as a key goal
• Conduct case conferences and coordinate care with service providers and outreach teams
• Collaborate with internal teams to ensure clients’ sustained success and prevent returns to homelessness
• Guide front-line staff in conflict de-escalation and crisis management
• Support inspections, audits, and adherence to compliance standards
• Perform other duties as assigned

Qualifications:

• LMSW (Licensed Master Social Worker) required (or must be obtained within six months of hire)
• At least 3 years of experience with similar populations; prior supervisory experience strongly preferred
• Solid clinical knowledge of service models: Housing First, Harm Reduction, Motivational Interviewing, and behavioral health systems
• Crisis intervention and risk assessment experience required
• Strong leadership, delegation, and team motivation skills
• Excellent communication skills (both verbal and written) with diverse populations
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams); experience with Access databases a plus
