Overview

The Clinical Coordinator is an integral part of the Healthcare Management team, working closely with the Care Management team to provide and support the implementation of person-centered clinical recommendations aimed at promoting the safety and well-being of complex members facing mental health, behavioral health, substance abuse, or forensic challenges. This role involves maintaining an active caseload for consultation, training, and direct care management support. Additionally, the Clinical Coordinator will assist in conducting occasional assessments and screenings, including specific psychological ID/DD and psychosocial assessments.

Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

Responsibilities

• Provide clinical oversight and person-centered recommendations that promote member safety and well-being.• Participate in discussions to ensure safe, person-centered interventions during discharge planning.• Act as a liaison between CDNY and community clinical resources to build collaborative relationships that support referrals for clinical services.• Advocate for individuals' access to resources and support, identifying systemic barriers within administrative and executive systems.• Conduct occasional ID/DD psychological and psychosocial assessments and screenings.• Adhere to professional standards outlined by protocols, rules, and regulations.• Maintain updated knowledge of relevant laws, regulations, and procedures.• Meet all training requirements on time.• All other duties as assigned.

Qualifications

• Minimum of 3 years working with individuals facing mental health, behavioral health, substance misuse, or forensic issues required.• Must hold a valid NYS license as a clinical social worker, master social worker, psychiatric mental health nurse practitioner, mental health counselor, or clinical psychologist required.• Master’s degree in social work, psychology (clinical or treatment), applied school psychology, nursing, or a science field with a clinical focus required.• Experience in conducting risk assessments, psychological or psychosocial assessments and screenings preferred.• Valid New York State Driver’s License required.• Ability to travel within New York State for occasional in-person psychosocial assessments and psychological ID/DD screenings that cannot be completed virtually required.• Professional or personal experience with individuals with intellectual and developmental disabilities required.
