Job summary

Enhabit, Inc. seeks a dedicated professional to manage referral-source relationships and drive post-acute care transitions.

You will educate physicians, hospitals, and other partners about Enhabit’s services and program requirements to support referral growth and patient outcomes. The role requires a Bachelor's degree in a healthcare field or nursing/therapy/social work background, with 2+ years in healthcare business development.

Qualifications

• Bachelor’s degree in healthcare admin, marketing, or related field; or nursing/therapy/social work degree.
• At least two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, and local Medicare home health and hospice regulations.

Responsibilities

• Serve as the primary relationship manager with referral sources (physicians, hospitals, health systems, etc.).
• Support referral growth by educating on services and program requirements to referral sources and patients.
• Help navigate post-acute care transitions to improve patient outcomes and satisfaction.
• Promote quality and satisfaction for patients and referral sources.
• In-office attendance is essential to achieve objectives (except for designated remote roles).
