You will lead treatment planning, budgeting support, group facilitation, and connect residents with Medicaid and community resources. The role emphasizes collaboration with a caring team in a welcoming environment.

Qualifications

• Bachelor’s degree in psychology, Social Work or related field.
• Experience within the mental health field a plus.

Responsibilities

• Provide case management including treatment planning, group facilitation, care coordination and skill development.
• Assist residents with budgeting and navigating Medicaid and other benefits.
• Develop the skills necessary to achieve greater independence and meet individualized goals.
