Qualifications

• Bachelor’s degree in Human Service field with 2 years full-time MH/DD/SAS experience, or 4 years in a related field, or RN with MH/DD/SAS experience, or Master’s with 1 year MH/DD/SAS experience.
• Must maintain strict confidentiality.
• Must possess effective communication/documentation skills.
• Excellent written and oral communication skills.
• Ability to work independently and as part of a team.
• Flexibility in responding to change.
• Demonstrated advocacy and negotiation abilities.

Responsibilities

• Participates in development/update of the Person-Centered Plan (PCP).
• Advocates for client and family participation in treatment and follow-up.
• Provides staff support for AP under Team Lead.
• Coordinates PCP activities under Team Leader.
• Performs case management and links to services and referrals.
• Documents billable encounters in the EMR within 24 hours.
• Maintains confidentiality of PHI and privacy.
• Completes Intake Packets for referrals.
• Participates in 24/7 on-call system for first response.
