Qualifications

• Bachelor's Degree in Nursing or Social Work.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• CCM certification a plus; continuing education required.

Responsibilities

• Coordinate plan of care with residents, families, and providers.
• Communicate Plans of Care and treatment protocols with residents and families.
• Attend morning stand-ups and facilitate team communications.
• Complete documentation and perform utilization review activities.
• Oversee admissions, discharges, and referrals to and from the facility.
• Maintain professional conduct and accurate charting.
