Job summary

UF Health in Jacksonville seeks a Case Manager to participate in the development of the patient’s plan of care, facilitate throughput, and conduct utilization reviews across the care continuum.

The CM collaborates with the patient and caregivers, Attending Physician, Utilization Review Coordinator, and other treatment team members to promote safe discharge planning and efficient resource use.

Qualifications

• Excellent verbal and written communication skills.
• Excellent organizational skills.
• Strong personal computer skills (i.e., Email, Word, Excel).
• Ty​ping speed 20 wpm.

Responsibilities

• Participate in development of the patient’s plan of care.
• Facilitate throughput and discharge planning.
• Conduct utilization review and promote appropriate utilization of resources.
• Collaborate with patient, caregivers, Attending Physician, Utilization Review Coordinator, and treatment team.
