An established industry player is seeking a Clinical Case Manager to join their Community Intervention Team in Madison. This role involves providing person-centered, trauma-informed care to individuals with mental health and substance use disorders. Position Summary
The Community Intervention Team (CIT) is a community-based case management program that serves adults with mental health and/or substance use disorders, supporting them with community living and independent living skills. The Clinical Case Manager will work with the Lead Case Manager and Community Case Management Supervisor to provide person-centered, trauma-informed care to a caseload of approximately 15-17 individuals and facilitate wraparound services to provide intensive and high quality care. Areas of focus may include symptom management, medication monitoring, housing, medical care, psychiatric care, crisis management, and benefits coordination.
