Qualifications

Job description

Join to apply for the Clinical Case Manager role at Centre for Neuro Skills.

The Clinical Case Manager, supervised by the Director of Rehabilitation, is responsible for developing, implementing, and managing all clinical aspects of the Centre for Neuro Skills. This role involves participating in decision-making processes that bridge clinical and administrative matters across both clinic and residential sites. The six core components of clinical case management include: processes and relationships, healthcare management, community resources and support, service delivery, psychosocial intervention, and rehabilitation case management. Work is full-time on weekdays (Monday-Friday, 7:30am-4:30pm) and may require on-call and overtime hours.

Job Tasks:
  • Gather information about the patient's situation and functioning to identify needs and develop a comprehensive case management plan.
  • Set specific objectives, goals, and actions to meet patient needs, ensuring plans are action-oriented and time-specific. Communicate and coordinate initial program issues and goals with staff.
  • Coordinate with external stakeholders and families to ensure service integration.
  • Implement case management activities and interventions to achieve plan goals.
  • Evaluate the effectiveness of the plan periodically, making modifications as needed.
Job Requirements:
  • Bachelor’s degree in a related field required; Master’s or licensure/credential preferred, but experience can offset educational requirements.
  • CCM certification preferred; successful completion required upon eligibility.
  • 3-5 years’ experience in rehabilitation or related fields.
  • Interest and capacity to learn about medical and clinical rehabilitation related to traumatic brain injury.
  • Supervisory experience is a plus.
