Job description

Join to apply for the Clinical Case Manager- RTC role at ACH Child & Family Services

Support youth in custody of TDFPS and their families by providing case management, medical/dental services, behavioral health supports, family communication, therapeutic services, crisis management, and transitional services within a trauma-informed care framework and regulatory standards.

Responsibilities include maintaining client files, developing assessments and service plans, coordinating resources, and working with clients and families to ensure safe living environments upon leaving the Residential Treatment Center. On-call support for clinical and program staff is also required.

Requirements:
  • Bachelor’s degree in social work, psychology, or related field; Master’s degree and/or certification preferred.
  • Experience varies by education: three years with a bachelor’s, one year with a master’s, including youth counseling, crisis intervention, intake, assessment, and case management.
  • Experience with adolescents and families in crisis, trauma-informed care, TBRI, and strength-based case management is preferred.
  • Excellent communication skills and basic computer knowledge.
Other Requirements:
  • Valid Texas driver’s license within 90 days of residency.
  • Acceptable driving record.
Working Conditions:

Exposure to children with behavioral issues, possible physical aggression, illness, and blood.

Responsibilities include:
  • Providing case management, family communication, aftercare, and transition planning.
  • Coordinating services with TDFPS, families, community partners, and staff.
  • Handling intake documentation, legal documents, and scheduling medical/dental/behavioral health services.
  • Participating in team meetings, documenting progress, and supporting communication with families.
  • Maintaining confidentiality, managing crisis situations, and ensuring quality services.
Qualifications:
  • Degree requirements as above; experience requirements vary accordingly.
  • Experience with crisis intervention, trauma-informed care, and youth/family counseling preferred.
