Job Description

Metrocare is the largest provider of mental health services in North Texas, serving over 55,000 adults and children annually. For over 50 years, we have provided a broad array of services including behavioral health care, primary care centers, veteran services, accessible pharmacies, housing, and supportive social services. Our researchers and educators at the Altshuler Center for Education & Research advance mental health beyond Dallas County and provide critical workforce training.

The QMHP-Dual provides psychosocial/rehabilitative and skill services to adults with serious mental illness and children with serious emotional disturbance. Responsibilities include assessment, developing measurable treatment goals, referral, linkage, advocacy, crisis intervention, and ensuring continuity of care. The goal is to maximize individual functioning, reduce hospitalizations, and support community reintegration.

Essential Duties and Responsibilities
  • Coordinate services, referrals, linkage, and advocacy for individuals with diverse needs, collaborating with an interdisciplinary team.
  • Conduct psychosocial assessments for new admissions.
  • Develop individual recovery plans based on assessments.
  • Set and assist in achieving measurable objectives with individuals.
  • Document efforts to involve individuals, providers, and caregivers in service planning.
  • Review recovery plans quarterly or as clinically indicated.
  • Implement recovery plans, ensuring objectives are met.
  • Engage and document interactions with caseload individuals.
  • Participate in team staffing, present assessment findings, and update on progress.
  • Ensure service authorizations are obtained.
  • Follow up on missed appointments and emergency service accesses within 24-48 hours.
  • Meet with individuals within 3 days of hospital discharge or community assignment.
  • Provide crisis intervention, including home visits and collaboration with police when needed.
  • Facilitate inpatient admissions upon team request.
  • Conduct individual, family, and group rehab sessions, evaluating progress and seeking feedback.
  • Assist individuals in obtaining entitlements through referrals and advocacy.
  • Perform other duties as assigned.
Competencies
  • Adhere to ethical standards, legal requirements, and professional guidelines.
  • Demonstrate analytical, problem-solving, and communication skills.
  • Meet adult and C&A competencies outlined by HHSC for TRR requirements.
Qualifications

Education & Experience: Bachelor's in human services with 1-3 years in MH/MR or related field, or Master's with 0-2 years experience.

Mathematical Skills: Basic math proficiency.

Reasoning Ability: Apply common sense to simple instructions; handle standardized situations.

Computer Skills: Proficiency in Word, Excel, Outlook, PowerPoint, internet research, and clinical record systems. Ability to organize workflow, manage caseloads, and communicate effectively. Flexibility to work independently and hours as needed.
