Humana Inc. is hiring a Clinical Care Nurse (RN) to join our clinic-based care team. You will support safe transitions of care, reduce avoidable ED utilization, and drive Medicare Advantage Stars and quality performance across value-based populations.

The role emphasizes data-driven quality improvement, patient education, and collaboration with providers and care teams to close care gaps, with daily in-center presence and travel within the market as needed.

Qualifications

• Active, unrestricted RN license in Mississippi.
• 3+ years of clinical nursing experience with transitions of care, quality improvement, or population health.
• Experience with EHRs and data analytics tools.
• Willingness to maintain Basic Life Support training.

Responsabilités

• Coordinate post-discharge follow-up to prevent readmissions and ED visits.
• Outreach to patients and providers to close care gaps and educate on preventive care.
• Document interventions accurately per CMS and organizational standards.
• Collaborate with interdisciplinary teams to implement evidence-based care and optimize workflows.
• Support quality improvement initiatives and center operations.
