Qualifications

• RN with 3+ years of clinical nursing experience
• Active, unrestricted RN license
• Experience with transitions of care and quality improvement
• Proficiency with electronic health records and data analytics tools
• Willing to complete Basic Life Support training

Responsibilities

• Analyze clinical data to identify care gaps related to Stars and HEDIS
• Coordinate timely post-discharge follow-up
• Educate patients and providers on preventive care and chronic disease management
• Conduct post-discharge outreach to confirm understanding of discharge instructions
• Collaborate with interdisciplinary teams to implement evidence-based interventions
• Document outreach efforts accurately and in CMS-compliant manner
