Humana Inc. seeks a Clinical Care Nurse (RN) to improve patient outcomes in a clinic setting. The role focuses on transitions of care, reducing avoidable ED visits, and boosting Medicare Advantage Stars.

You will balance direct patient education with data-driven quality work and support a compassionate, equity-centered care model. Responsibilities include post-discharge follow-up, care coordination with care teams, and participation in quality improvement projects.

Qualificações

• Must have ADN or BSN nursing degree.
• Active, unrestricted RN license.
• 3+ years of clinical nursing with transitions of care or population health.
• Experience with EMR/data tools and MS Office.
• Ability to complete Basic Life Support training.

Responsabilidades

• Support TOC and post-discharge follow-up to prevent readmissions.
• Analyze data to identify gaps in Stars and HEDIS measures.
• Proactive outreach via phone/telehealth/in-clinic visits.
• Educate patients on preventive care and chronic disease management.
• Collaborate with interdisciplinary teams and document outcomes.
• Participate in quality improvement and center huddles.
