• 3+ years of clinical nursing with TOC/quality impact.

Responsibilities

• Analyze clinical data to identify gaps in care related to Stars/HEDIS.
• Coordinate post-discharge follow-up to prevent readmissions.
• Outreach to patients and providers via phone/telehealth/in-clinic visits.
