Job summary

Housing Works is seeking a Clinical Care Manager for the BEW project in partnership with NYC DOHMH. You will assess client needs, coordinate care, develop health goals, and promote retention in care for HIV-positive women, with emphasis on high-risk populations and cultural competency.

Qualifications

  • MSW or BA/BS with ≥5 years care management experience.
  • Cultural competency with Black and Latina populations.
  • Experience completing assessments and developing care plans.
  • Experience documenting in electronic records.
  • Ability to work in a team and interdisciplinary system.
  • Knowledge of HIV/AIDS, homelessness, substance use and mental illness.
  • Bilingual preferred (English/Spanish, English/Haitian-Creole) but not required.

Responsibilities

  • Facilitate enrollment in BEW program and assist with intake, outreach, eligibility.
  • Conduct initial needs assessments and collaborate on care plans.
  • Conduct reassessments and update plans at regular intervals.
  • Verify engagement in HIV treatment and reconnect to care as needed.
  • Collaborate with interdisciplinary team to deliver health education sessions.
  • Provide direction to Client Navigator and assist access to health and social services.

Compensation Range: $21/hr - $24/hr commensurate with experience
Benefits:

Overview:

The BEW(BeIntoHealth forWomen)Project is a partnership between Housing Works and NYC DOHMH, supports Black and Latina women living with HIV in reengaging in HIV care and essential support services. The Clinical Care Manager works with an interdisciplinary team to assess client needs, coordinate care, develop health goals, and promote retention in care and self-management. The ideal candidate has experience serving high-risk populations and strong cultural competency working with diverse gender identities and communities.

Responsibilities:
  • Facilitateclient enrollment in BEW program including assisting withintake, outreach/engagementand eligibilityverification.
  • Conduct initial needs assessments andcollaboratewith clients on establishingachievablecare plan goals based onidentifiedneeds.
  • Conduct reassessment and update plans of care at regular intervals.
  • Verify engagement in HIV treatment,providetimelyreconnection to care.
  • Collaborate withthe interdisciplinaryteam on the delivery of individual and groupstructuredhealth education sessions.
  • Provide direction and support to the Client Navigator in the delivery of enhancedclient navigation services,includingassistancewith access to health care, socialservices, entitlements, orotherservices that reduce barriers to engaging in healthcare.
Requirements:
  • Master Social Worker (MSW,LMSW)or BA/BS with no less than 5 years care management or navigation experience.
  • Cultural competency with Black andLatina, Cisgender,transgender, on-binary orgenderqueer women.
  • Prior experience with completing assessments and developing care plan goals.
  • Experience with documentation inelectronicrecords
  • Abilityto work in a team management and interdisciplinary system
  • Understanding of HIV/AIDS and its impact onhomelessness, substanceuse and mental illness
  • Bilingual (English/Spanish, English/Haitian-Creole) preferred but notrequired.
