Job summary

Pride Health in Phoenix, AZ is seeking an experienced Registered Nurse with Case Management experience to join our patient-centered team. The RN will coordinate care, advocate for members, and work with providers to arrange referrals, diagnoses, and services across the care continuum, ensuring timely, high‑quality care both in person and remotely as needed.

Schedule is Monday–Friday, 8:00 AM–5:00 PM, onsite.

Qualifications

  • RN license in the practicing state
  • CPR certification required
  • Case Management or Care Management experience preferred
  • Experience with Electronic Medical Records (EMR/EHR)
  • Customer service oriented with strong patient advocacy and communication skills

Responsibilities

  • Coordinate care and referrals with physicians and specialists
  • Assist with chronic condition management and patient education
  • Advocate for patients and guide them through the healthcare system
  • Coordinate scheduling and office processes to ensure timely services
  • Support care team with referral information and status updates
  • Participate in quality improvement initiatives at the center
  • Build relationships with providers and community partners

Work Arrangement: 100% Onsite

  • Sign-On Bonus: $80,000.00 - $90,000.00 annually
  • Up to a 10% annual bonus
  • $5,000.00 Sign-on bonus
Position Overview

We are seeking an experienced Registered Nurse to join our team in Phoenix, AZ. This role is ideal for an RN with Case Management/Care Management experience who enjoys working directly with patients, coordinating care, and serving as a strong advocate throughout the healthcare journey.

The RN will support members both in person and virtually, coordinate specialist and diagnostic referrals, assist with chronic condition management, and help ensure patients receive timely, high-quality care.

Key Responsibilities
  • Support members with real-time consultation, education, scheduling, and coordination of specialist and diagnostic appointments.
  • Manage the referral process from beginning to end, including follow-up and ensuring completion of recommended services.
  • Explain specialist and diagnostic recommendations clearly and assist members in navigating the healthcare system.
  • Serve as an empathetic and highly visible advocate for patients and members.
  • Support improved health outcomes and help identify opportunities for cost savings.
  • Lead patient outreach initiatives related to gaps in care and other clinical needs.
  • Support members with chronic conditions through education, reminders, and ongoing care coordination.
  • Assist with the management and integration of office processes.
  • coordinate service and scheduling requests on behalf of members.
  • Provide the Care Team with referral information, requirements, and status updates.
  • Help develop and manage quality improvement programs at the center.
  • Build strong relationships with healthcare providers, social/community organizations, and other local partners.
  • Sere as an ambassador for the center within the local community.
  • Perform other duties as assigned.
Required Qualifications
  • Current Registered Nurse (RN) license in the state of practice.
  • CPR certification required.
  • Case Management or Care Management experience.
  • Electronic Medical Records (EMR/EHR) experience.
  • No specific EMR/EHR system experience is required.
  • Customer service-oriented with strong patient/member advocacy skills.
  • Excellent interpersonal and communication skills.
  • Strong communication skills across multiple platforms and modalities.
