Job description

The Clinical Care Coordinator- Certified is the key liaison between the patient and their Patient-Centered Medical Home team. The team includes providers, clinic staff, and other health care professionals. The Clinical Care Coordinator works to ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.

Required Qualifications:
  • Completion of an accredited Medical Assistant program.
  • National Provider BLS - American Heart Association upon hire.
  • 3 years of recent health care experience working as a Medical Assistant.
  • Specialty areas: 6 months of recent experience working as a Medical Assistant.
Preferred Qualifications:
  • College degree.
