Job Description

The Clinical Care Coordinator - Certified serves as the key liaison between the patient and their Patient-Centered Medical Home team, which includes providers, clinic staff, and other healthcare professionals. The coordinator ensures safety, adherence to best practices, and high-quality standards are maintained for the patient throughout the healthcare continuum.

Required Qualifications
  • Completion of an accredited Medical Assistant program.
  • National Provider BLS certification from the American Heart Association upon hire.
  • At least 3 years of recent healthcare experience working as a Medical Assistant.
  • Recent experience (6 months) in a specialty area as a Medical Assistant.
Preferred Qualifications
  • College degree (preferred).
